Submit request online using form at www.Lindtusa.com/replace-it by providing all required information, including copy of receipt with date of purchase and store name and address. Requests must be submitted within 30 days of purchase. Physical $4 voucher will be sent via US Mail to address provided. Voucher expires: 12/31/2024. Voucher redeemable at selected stores. See voucher for details. Offer limited to US residents only, 18 years of age or older. Limit one request/voucher per person or address. Use of multiple addresses or P.O. boxes to obtain additional vouchers is fraud and may result in prosecution. Multiple submissions will not be acknowledged or returned.
